You should have a supply of DOT exam forms and the medical certificate on hand at all times. Often, motor carriers will have the drivers bring in their own modified forms. You can download them all right here:

Have a question if I am registered at one facility to do DOT exams how can I add another site in different county to do DOT exams there as well. I have tried to get this answer for months and hope someone can help me
