Which Forms Do I Use for the DOT Driver Exams?

Certified Medical Examiners are required to continue to use the Medical Examination Report Form and Medical Examiner’s Certificate as found on the Federal Motor Carrier Safety Administration website at http://www.fmcsa.dot.gov/medical/driver-medical-requirements/medical-applications-and-forms. These forms will be in effect until December 22, 2015.
Beginning December 22, 2015, Certified Medical Examiners are required to use the new Medical Examination Report (MER) Form, MCSA-5875 and Medical Examiner’s Certificate (MEC), Form MCSA-5876. FMCSA will update the CMV Driver Medical Examination Results Form, MCSA-5850 in the National Registry to allow you to enter the MEC information that will populate the new MEC, Form MCSA-5876.

When Must I Report “No Exams Performed”?

Certified Medical Examiners are required to report “no exams performed” during the previous month by the last day of the month. For example, if you did not perform any DOT CMV driver exams during the month of April, you must report that by May 31st. This is now a regulatory requirement.
